Request a Report

Medical Examiner reports are typically available 90 days after the death investigation and/or postmortem examination takes place. Please complete the electronic submission form below, and submit payment via credit card through online payments, or mail a check or money order to:

Report cost:

$10 for emailed PDF (suitable for most purposes)
$35 for a certified report sent via US mail (usually only needed for court proceedings)

(There is no fee required for reports for law enforcement, government agencies, or Medicaid/Medicare patient providers. Please note your affiliation in the “Relationship to Decedent” field below.)

Gila County examination reports are available through the Pinal County Medical Examiner if the death occurred on or after July 1, 2020.

If you are requesting a report for a Gila County Case for a death that occurred prior to July 1, 2020, please contact the Gila County Attorney's Office at 928-402-8630. Please do not complete the below form or pay the fee if the Gila County death occurred before July 1, 2020.

To request the examination report for a death that occurred within Yuma County, please contact the Yuma County Sheriff's Office Medical Examiner Division via telephone at 928-783-4427. Please do not complete the below form or pay the associated fee for Yuma County Deaths.

STATEMENT OF NON-COMMERCIAL PURPOSE

Caution: Arizona Revised Statute 39-121.03 (C) provides:

A person who obtains a public record for a commercial purpose without indicating the commercial purpose or who obtains a public record for a noncommercial purpose and uses or knowingly allows the use of such public record for a commercial purpose or who obtains a public record for a commercial purpose and uses or knowingly allows the use of such public record for a different commercial purpose or who obtains a public record from anyone other than the custodian of such records and uses it for a commercial purpose shall in addition to other penalties be liable to the state or the political subdivision from which the public record was obtained for damages in the amount of three times the amount which would have been charged for the public record had the commercial purpose been stated plus costs and reasonable attorney fees or shall be liable to the state or the political subdivision for the amount of three times the actual damages if it can be shown that the public record would not have been provided had the commercial purpose of actual use been stated at the time of obtaining the records.

I hereby agree that the public records I have requested are not for a "commercial purpose" as defined by A.R.S. § 39-121.03, and will not be resold to any person at a cost based upon the value of the information contained in the public records.
